Documents that make your visit faster
Always bring
- Your national ID or passport
- Your Mother & Child booklet or ANC card
- Previous clinic cards, scan reports and laboratory results
- Your SHA or insurance details, if you have cover
If you were referred
- Bring the referral form from your Level 1–3 facility intact
- It sets your service category and keeps your records continuous
- Ask for a copy of any results taken before referral
- Referred patients are attended to in the relevant specialist clinic
First antenatal visit
- Come as early as possible — ideally before 12 weeks
- Bring any previous pregnancy records or test results
- You can be registered and started on the same day
- Ask about the ANC exercise and nutrition guidance on your first visit
Before you leave: the paperwork you will need
The Health Records office prepares the documents families need after a birth or a clinic visit. Ask at Reception for the records desk and collect what applies to you while the details are still fresh.
Notification of birth
- Births that take place at Pumwani are notified through the Health Records office
- Collect the notification of birth from the records desk before you go home
- It is the first document required when applying for a birth certificate at the civil registrar
- Bring the mother’s national ID, with the father’s details where available
Discharge summary
- A written summary of your care, procedure, medicines and follow-up
- Take it with you to your postnatal or follow-up clinic visit
- Keep it for SHA or insurance claims and for treatment elsewhere
- Ask the ward nurse to explain anything unclear before you leave
Child immunisation card
- Your baby’s first immunisations are recorded on the card before discharge
- Bring it to every immunisation, growth-monitoring and clinic visit
- The card carries your child’s record between facilities nationwide
- If it is lost, ask the records desk about a replacement
Copies of your records
- Delivery summaries, laboratory results and imaging reports
- Request them at the records desk with your national ID and visit date
- Allow a few working days; a fee may apply for copies
- Useful for SHA claims, school and referral requirements
